My animal has never harmed myself, another individual, or another animal, and has not demonstrated any aggressive behaviors. I understand I am responsible for my animal’s behavior and take full responsibility for any damage or harm they may cause.
I understand that if my animal attacks me, another individual, or another animal, I alone am responsible for any legal or financial consequences, and I will not hold K9 Training Institute, or the therapist arranged by K9 Training Institute, responsible.
I understand that K9 Training Institute, and my therapist are not qualified to evaluate my animal's behavior, and my animal is entirely my responsibility.
I understand that an emotional support animal (“ESA”) or psychiatric service dog (“PSD”) is not a Service Animal, Therapy Animal, or Guard Animal.
I understand that if approved for an ESA, my animal may accompany me in my dwelling but may not be permitted in public places where animals are forbidden.
I agree to complete the assessment thoroughly and in detail, avoiding one-word answers.
I understand the differences between ESAs and PSDs, and their respective training requirements.
I confirm that I am physically located in the state listed in the address I have provided. (If not, please contact us to update your information to match your location, as therapists cannot evaluate patients across state lines.)
I understand that editing the therapist’s support animal recommendation letter in any way is considered forgery, identity theft, and fraud.
I agree not to alter the recommendation letter issued to me by my therapist.
I understand that forgery is a felony in the United States, and altering any aspect of a document provided by my therapist may result in dismissal from treatment and reporting to law enforcement. This includes allowing another individual to alter my recommendation letter for their own use.
